The Site is intended for adults (18+) seeking information about our services, employment, or general inquiries on behalf of a family. You are responsible for the accuracy of information you submit.

Content on the Site (including blog posts, FAQs, and service descriptions) is for general information. It is not medical advice, a diagnosis, a guarantee of outcomes, or a substitute for professional evaluation. Therapy recommendations are made only after appropriate clinical assessment.

4. No guarantee of services or insurance coverage

Submitting a form does not create a treatment relationship or guarantee availability, insurance coverage, or authorization. Coverage depends on your plan, network rules, staffing, and clinical eligibility. We may need additional information before we can confirm next steps.

5. Website forms and prohibited content

Contact form: For general questions only. You must not include PHI or other sensitive personal details (child’s full clinical history, diagnosis details beyond a high-level inquiry, insurance IDs, SSNs, etc.).

Enroll / secure form: Use this channel for enrollment-related information intended for our team.

Careers form: For employment applications and resumes only.

You agree not to use the Site to submit unlawful, harmful, harassing, or misleading content, or to attempt to disrupt or probe the Site.

6. No spam; communications

We will not spam you. By contacting us, you consent to us responding about your request. Marketing emails, if any, are opt-in only and can be stopped on request.

To the fullest extent permitted by Arizona law, Oasis Therapy Group and its team are not liable for indirect, incidental, special, consequential, or punitive damages arising from your use of the Site. Our total liability related to Site use will not exceed one hundred dollars (US $100), except where liability cannot be limited by law. This does not limit liability for clinical services governed by separate agreements and professional standards.

11. Indemnity

You agree to indemnify and hold harmless Oasis Therapy Group from claims arising out of your misuse of the Site or your violation of these Terms, to the extent permitted by law.

12. Governing law

These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-law rules. Courts in Maricopa County, Arizona, will have exclusive jurisdiction for disputes arising from these Terms or the Site, except where prohibited.

13. Changes

We may update these Terms at any time. The “Last updated” date will change when we do. Continued use of the Site after changes constitutes acceptance.
